Main outcome of the role:

Hospital pharmacists are considered a core member of the clinical care team, when it comes to delivering patient care.

The primary objective of their work is to ensure that patients experience safe medication use.

Beyond that purpose, however, the role of hospital pharmacists encompasses a broad range of duties with the ultimate goal of providing quality care during an inpatient stay, ensuring a seamless transition of care, and reducing the number of medication errors

Hospital pharmacist job includes determining which form of medication best suits each patient, monitoring the effects of the medications prescribed and counsel patients on the effects of the drugs, recommending administration routes and dosages, all of which are dependent on an individual's needs.

The hospital pharmacists verify orders for appropriateness before proceeding compounding, or preparation of medication and dispensing to patients, this includes prescriptions for inpatients (unit dose), out patients (home medications), narcotics, electrolytes and chemotherapy …

The hospital pharmacists are source of information regarding medications and its use, providing clinical expertise and being an educational resource for colleagues and patients and provide guidelines and policies of using medications all over the hospital.

Key Responsibilities:

  1. Check prescriptions for errors, ensuring they're appropriate and safe for the individual patient
  2. Responsible for the accurate dispensing and timely distribution of medications by daily preparing the unit dose for all in patients.
  3. Ensuring that each patient got the right drug with the right dose, the right form and the right time.
  4. Provide advice on the dosage of medicines and the most appropriate form of medication for other health care providers.
  5. As clinical pharmacist to attend medical rounds and discussion of patient’s medications and routinely provide clinical interventions to improve the health outcomes of patients.
  6. Preparing guidelines for use of medications at the hospital.
  7. Dispensing and controlling narcotics as per the ministry of health regulations
  8. Preparing chemotherapy agents and cytotoxic material as requested following cytotoxic safety handling procedures.
  9. Provide information on medications for health providers.
  10. Counseling and educating patients on medication use and side effect.
  11. Ensure medicinal products are received and stored appropriately and securely to ensure potency. {Regular checking of room and refrigerator’s temperature and expiry dates of all medications}
  12. Preparing the medical unit’s needs of mini-stock medications and checking their stores on a monthly base.
  13. Dispensing out patient’s prescriptions upon discharge.
  14. Prepare and quality-check sterile medications, for example, intravenous medications (electrolytes and anti-cancer drugs).
  15. Responsible for inventory at the pharmacy and stock level of medication
  16. Observing hospital and pharmacy policies in relation to medications and enforcing them.
  17. Fulfill pharmacy work on electronic medical system and pharmacy forms and log sheet.
  18. Dispensing instruments for patient’s operations and/or procedures.
  19. Supervising and checking the work of the less experienced and less qualified staff.
Responsible to medication-related inquiries from within the hospital
  1. Communicating effectively with other health care staff (physicians and nurses)
  2. Doing what is requested by his/her supervisor in all aspects regarding quality work at the pharmacy.

Required Qualifications, Skills and experience : 

  1. BA degree in Pharmacy or pharmacy doctor from a credited university.
  2. Having a valid Pharmacist license to work in Jerusalem.
  3. RECOMMENDED At least one year (of experience as a Pharmacist with relevant experience and skills in calculations and analyzing of information, good judgement skills, Good knowledge of MOH health regulations, experience in Pharmacology, ability to focus on work quality and give great attention to details. 
  4. Effective and strong communication skills.
  5. Ability to work independently under pressure, meet tight deadlines, and demonstrate flexibility and creativity in planning and problem solving.
  6. Proven ability to take initiative, learn quickly, and be accountable for results.
  7. Excellent interpersonal skills and strong personnel management skills.
  8. Able to work collaboratively and inspire learning based on experience and to achieve results.
  9. Fluency, writing and reading in English and Arabic, Hebrew is a plus.
  10. Respect, Promote Ethical values and human rights
  11. Maintain professional relationships and act as guides and advocates.
